Modernize a legacy system in stages you can verify
A legacy system may still perform its core job while making people re-enter data or search through disconnected records. Begin with a bounded problem rather than assuming the entire system must be replaced.

Map the constraints
List systems, data owners, dependencies and steps that cannot stop. Review documentation, access and backups before designing the change.
Define the first deliverable
Choose one workflow, such as reducing manual copying between systems. Agree inputs, outputs and acceptance criteria. Make the retained and changed parts clear to users.
Plan transition and handover
Define the transition window, data comparison, rollback triggers and decision owner. Test with users within an approved scope, then deliver operating instructions and ownership before expanding.
